The shock wave is a discontinuous mechanical wave with the wave front time of nanoseconds, wave width of micrometers and pressure of hundreds of megabars from atmospheric pressure to peak. The response pattern of osteoblasts after shock wave treatment was observed in animal experiments in 1980, which aroused people's interest in the application of extracorporeal shock wave therapy (ESWT) in the treatment of musculoskeletal diseases [1,2]. In 1997, ESWT was reported to treat orthopedic diseases such as tennis elbow and pseudoarthritis. Currently, shockwave therapy has become an option for the treatment of many musculoskeletal disorders, including plantar fasciitis, external epicondylar disease of humerus, knee osteoarthritis, delayed union and nonunion of long bone fractures, patellar tendinitis, Achilles tendinitis, and avascular necrosis of the femoral head [3,4,5,6,7,8,9]. It has been recognized in Europe (Germany, Austria, Italy, etc.), South America (Brazil, Colombia, Argentina, etc.), Asia (South Korea, Malaysia) and North America (the United States, Canada). However, the clinical application of ESWT is still lacking of standardized guidelines and in-depth scientific research. Therefore, we organized some experts in the field of physical medicine and rehabilitation in China to systematically retrieve clinical research data, evaluate the quality of evidence, and discuss and formulate ESWT expert consensus for musculoskeletal diseases by applying evidence-based medicine methods. The physical nature of shock waves
Shock wave is a discontinuous mechanical wave with very short wavelength and strong energy propagating in the medium, which is characterized by the speed of the medium exceeding the propagation speed of the wave in the medium. The pressure of the first quarter of the wave is called the wave front, and its medium density gradually increases under the action of high pressure.
